CV Builder Instructions
Curriculum Vitae (CV) is Latin for “course of life”. It provides a summary of a faculty member’s experience, academic background including teaching experience, degrees, research, awards, publications, presentations, and other achievements, skills and credentials.
It is a summary of information about a faculty that is typically publicly visible.
Having a common format for CV’s allows those at the university to review in a more efficient and consistent way.
It ensures that common information is included and facilitates review and interpretation.
Faculty are encouraged to maintain a current Yale CV. In addition to making it available as needed, periodically updating it can ensure that information is captured and allow faculty and mentors to track progress.

CV Builder Tool
The Yale Profile System that faculty use to create their online profile has a CV Builder Tool that generates YSM-standardized Appointments and Promotions CVs.
While the tool is in beta testing anyone at the school is welcome to use it to generate a Yale CV.

General Instructions and Introduction
Information Included
CURRICULUM VITAE should be the label of the document.
Date of Revision: MM/DD/YYYY
Name: Full name, degrees (no periods)
Proposed For:
Appointment, reappointment, or promotion
Full title, including rank, department, and track
Secondary/coterminous appointment, if applicable
Term:
For term appointments: MM/DD/YYYY to MM/DD/YYYY
For Professors in the Clinician-Educator, Clinician-Scientist, and Investigator Track:
On a continuing basis beginning MM/DD/YYYY
For Professors and Associate Professors with Tenure in the Traditional Track:
With tenure beginning MM/DD/YYYY
If coterminous:
One of the above designations
Plus: coterminous with (YNHH, VAMC, Pierce Foundation) appointment
School: Yale School of Medicine (and the Graduate School, if applicable)
Formatting
There are a number of relatively minor modifications on this version of the instructions compared to prior.
Header should have faculty name with degrees (upper right)
Footer should have page numbers (centered)
CV Builder
CV builder is part of the Profile System used at YSM
Once data is entered into this system, it will populate faculty profile pages on the YSM website and can be used to generate CVs
Each of the following sections of these instructions indicates how information can be entered to populate the different sections of CVs generated by CV builder

Information Included
YYYY, degree (no periods), major/department/other qualifier, School, city, state
Begin with undergraduate degree and list in chronologic order
If the CV is for YM credentialing purposes, you must include the month and year (MM/YYYY)

Information Included
YYYY-YYYY of position/appointment (end date can be "-present" if applicable)
Role, such as:
Internships, residencies, chief residency, postdoctoral fellowships, research faculty positions (e.g. Associate Research Scientist)
Teaching/ladder faculty positions (e.g. Assistant Professor)
Department (if applicable), organization/institution, city, state
List in chronologic order
If the CV is being used for YM credentialing purposes, you must include the month and year (MM/YYYY)

Awards or honors from organizations, election to honorary societies or to limited membership societies, and fellowships in professional organizations

Information Included
Grant and clinical trial history should be separated into the following sub-categories:
Current
Past
Pending (should already have been submitted and a preliminary score should be provided if available)
The description of each grant and clinical trial should include the following
Agency: Name of funding organization, kind of grant or clinical trial
ID#: Organization ID (Yale IRES number for current and pending grants/trials)
Title: Title of grant
P.I: Name, degree
Role on project: If not principal investigator
Percent effort: note that total % effort on active grants and clinical trials must not exceed the % effort toward research listed on the CV Supplement
Direct costs per year: active grants only
Total costs for project period
Project period: MM/DD/YYY-MM/DD/YYYY

Invited lectures and faculty service in CME courses and workshops from organizations not affiliated with Yale
List educational materials developed specifically for these presentations, such as syllabi for review courses and computer, multimedia or web-based projects
Do not include in this section:
Presentations given at Yale University/Yale School of Medicine/Yale New Haven Health System
Presentations in which your invitation arose from the submission of an abstract or topic for consideration (those items should be listed in peer-reviewed presentations below)
Specify date, location, title of presentation and sponsoring organization
