Forms
Stakeholders should develop their own forms either using Microsoft Forms or Qualtrics, both are available and supported through Yale. Qualtrics is recommended when HIPPA compliance or complex logic are required. Microsoft Forms is recommended for simple forms.
Simple forms can be embedded into an iframe. CTAs or Links will be used to more complex forms or pages.
Best Practice: It is important that any form be shared with other members of your work group to ensure that staff turnover does not compromise data or access to it. We also recommend sharing with a generic email address.
Qualtrics Form and Survey Builder
Qualtrics is a survey and analytics tool, and provides over 100 different ways to ask a question. This opens up many possibilities and further flexibility for Yale School of Medicine’s users.
Accessing Qualtrics
Visit the Yale Qualtrics Homepage by logging into the Central Authentication Service (CAS) window that opens using your Yale NetID and password. Upon initial login, complete the short form with your preferences for first and last name. Some schools and departments have specialty Qualtrics sites with different websites.

Getting your form on to a YSM website
When your form is complete and ready to be distributed, please contact ysm.editor@yale.edu and submit a detailed request with your link for distribution and our team can work with you to establish the best method for promoting the form or survey.
